[tree-ssa] preliminary passes.texi rewrite

I decided that this should be closer to a design document than
documenting what currently exists.  What currently exists is so
incoherent that its no wonder that the documentation is crap.
I'll be rewriting the code to match as we decide what it is we
want to do.

I'm uncertain how much detail we want to put here.  Should this
be more of an overview and the details of the front-end and
optimization pass APIs be placed elsewhere?

Anyway, here's what I have as of today.  Folks should feel free
to send me better blurbs for their optimization passes.
