PATCH RFA: Expose demangler internal interface for use by gdb

This patch exposes the demangler internal interface for use by gdb.
Or anybody else who is interested, I suppose.

The new interface is defined in include/demangle.h. The bulk of the
patch consists of renaming things to use names which are reasonable
for a public header file.

This patch may be slight overkill, in that I didn't want to clutter up
the basic cp-demangle.c with functions which are only supplied for the
new interface, and are not required merely in order to demangle
names. In order to accomplish this, I created two new files in
libiberty. cp-demint.c provides the functions which are needed for
the interface but are not used by the demangler. cp-demangle.h is a
private header file shared by cp-demangle.c and cp-demint.c.

The functions in cp-demint.c are not well tested. Reported bugs will
be fixed promptly.

Ian
