[libstdc++] more explicit instantiations

As mentioned on libstdc++ list. Tested on i686-linux and powerpc-eabisim.

PR libstdc++/3829
* src/misc-inst.cc: Many additional I/O-related instantiations.
* testsuite/27_io/instantiations.cc: New file.
