Target FUNCTION_PROLOGUE and FUNCTION_EPILOGUE

This patch, against current CVS, is too long to post without gzipping
it.  It moves these two macros to our new target structure.

I have bootstrapped and made check x86 Linux.  Further, I have
compiled the following through stage1, and ensured I have created no
new warnings.

There are some others I intend to check before I commit this patch.

I have not been able to fully check dsp16xx, elxsi, romp, i960 or ia64
because of reasons other than this patch (ICEs, broken compilations,
etc.).

Documentation changes are included.  I hope my changelog for the config
directory is OK; it would be unbelievably tedious to document each
repetitive similar change for the 250K of diffs it comprises.

	in cpu/cpu.c, or rename old functions consistently.  Take
	a HOST_WIDE INT not an int as the SIZE parameter.  Remove now
	redundant macros and prototypes.  Make new functions static.
