I think the problem here is not so much that it's difficult to implement but
rather that there isn't enough interest or motivation in providing a solution
(which is evident from the absence of updates/patches/duplicate requests since
2008). IMO, the appropriate way to solve the problem is for projects/users to
define a macro in the user namespace (no leading underscores). With that,
resolving as Won't Fix.
