There are several problems with the version of time_put::put that takes
a format string as an argument.

1. The fill character is not passed to do_put.

2. Characters that are not format specifiers are not written directly to
the output iterator, but passed through ctype::narrow first.

3. put reads past the end of the format string if it ends in a single '%'.
