Subject: [v3] doc updates

        * doc/xml/manual/extensions.xml: Improve markup and note that some
        extensions are included in C++11.
        * doc/xml/manual/concurrency_extensions.xml: Likewise.

Tested with 'make doc-xml-validate-docbook doc-html-docbook'
