Done. I've checked in the attached patch and regenerated the html
files, I hope they're OK.   I made one additional change, in
appendix_contributing.xml I changed the "Bash 2.x" requirement to
"Bash 2.0 or later" since Bash is up to 4.0 now and that works fine.
