Subject: [v3] normalize basic_filebuf data qualifications

Some of the basic_filebuf data members are not qualified, some are
over-qualified. To avoid confusion, I've normalized this WRT other
classes in iostreams, and done a preliminary review of the other
iostream classes. 

I suspect this is just an oversight from some of last year's work, where
these classes were massaged.
