Subject: [v3] saner defaults for __per_pool_pool_policy's __pool_type

... as per commentary in libstdc++/16614. 

All I'm going for are sane defaults that fix the glaring issues.
Specific tuning is already possible with __pool_base member functions.

I believe this fixes up the last of the known issues. I'd like to do
another round of performance tuning on this sucker now that these other
issues have been addressed.
