My advice is to get the form done ASAP.  It can take a long time.
By the time you get the paperwork done, the contributor may have
moved on to other things, and no longer be able to provide their
patches or whatever.  That's what happened to my work.  The legal
bullshit took so long, that my project was over, and I no longer
could justify the time to unbury the dead issue and produce nice
topical patches for submission.
