I believe one reason is that people will sometimes use the wrong form.
Another reason is that they have a relatively new system where
the FSF prints out the appropriate form, and mails it to you
for you to sign. The advantage is that then the FSF doesn't have
to check that the form is the standard form or if you've changed
something. This saves people-time, even if takes more real-time.
