I've been thinking that maybe we could build a little web form that would
quiz the contributor, to help us figure out what form that person needs.
We still wouldn't publish the forms themselves; after all, two physical
pieces of paper have to be signed by both the FSF and the contributor, so
it's fine to have the FSF sign first and send the paper.
