PATCH COMMITTED: Add TV_NONE, use it for timevar values


With my updated -Wc++-compat patch which is under development, there are
a few places which warn about passing an integer to a function which
expects a timevar_id_t.  I think the cleanest way to fix this going
forward is to change the relevant structures to have fields of type
timevar_id_t rather than int.  This then requires changing a bunch of
initializations to use an enum value.  Previously the passes used the
value 0 == TV_TOTAL, and relied on the fact that no pass actually wanted
to specify TV_TOTAL.  Rather than continue that, I added a new enum
constant TV_NONE, and changed the initializations to use that.

This mostly mechanical patch was bootstrapped and tested on
i686-pc-linux-gnu.  Committed to mainline.
