RFA: Fix demangler testsuite and a couple of demangler buglets


In an attempt to avoid some problems I've already had, I beefed up the
demangler testsuite to test demangling without DMGL_PARAMS, and to
test is_gnu_v3_mangled_ctor() and is_gnu_v3_mangled_dtor().

The improved testsuite then uncovered a couple of minor problems in
the V3 demangler and in the demangling of HP and ARM templates.

This patch is for the upgraded testsuite, and includes the demangler
patches.  With these patches, all the tests still pass.  The only
testsuite changes are for the case without DMGL_PARAMS, which was not
previously tested, and a few new constructor/destructor identification
tests.

I'll wait for approval to check this in, since it affects more than
just the demangler code which I wrote.

	CV-qualifiers from D_COMP_LOCAL_NAME right subtree.
	* cplus-dem.c (demangle_arm_hp_template): Set DMGL_PARAMS when
	demangling template parameters.
	* testsuite/test-demangle.c (fail): New static function.
	(main): Support new options in input file: --no-params,
	--is-v3-ctor, and --is-v3-dtor.
	* testsuite/demangle-expected: Add --no-params to most tests, and
	add the correct result when parameters are not demangled.  Add
	some simple tests for V3 constructor/destructor recognition.
