[PATCH] Fix PR target/5626 (take 2)

Hi!

The following patch fixes target/5626 (thanks to Brad Lucier for
bootstrapping/testing this).
I haven't changed the brCC handling much since last patch, since
a) although %{i,x}cc live accross such branches is rare, IMHO it is
   far rarer to have extremely huge routines
b) it would be inconsistent with bCC %xcc and fbCC %fcc{1,2,3},
   since theer it cannot be solved this way
c) as brCC do 64-bit comparisons, the 2 insn replacement (instead of 3)
   would be limited to +-256kinsn anyway, so another solution would
   need to be used for even longer branches
This patch converts bCC %icc and fbCC %fcc0 very far branches into
the v8 style ones (bCC and fbCC; one for one insn, the drawback
is losing the initial prediction) and brCC, bCC %xcc and fbCC %fcc{1,2,3}
into a 3 insn sequence.
There are some CCX_NOOVmode handling bugfixes in the patch too.
Richard, do you agree?
