GFortran documentation updates merged into 4.2.

- Subject: GFortran documentation updates merged into 4.2.
I just merged all of my recent GFortran documentation changes and
updates into the 4.2 branch. Rather than attach the quite large diff
between the pre- and post-commit versions, I'm attaching the much
smaller diff between the current 4.2 and Mainline versions of the
documentation files. This should cover all of the documented
differences between the two branches -- at present, this is a handful of
F2003 features, and four new compiler options.
The merged files were tested with "make pdf" and "make info" on
i686-pc-linux-gnu, and committed under blanket approval from Steve Kargl.
