(cast_expr): Use new initializer parsing functions.

(initdcl, notype_initdcl): Likewise.
(init): Likewise.
(initlist_maybe_comma, initlist1): These replace initlist.
(initelt): New nonterminal.
Change specified index syntax to `[INDEX]='.
Change specified field syntax to `.NAME='.
