[v3] numeric facets cleanup

Long threatened... this is now in a state where it is not causing
regressions. I've still got to do some work on it, but tightening and
tracking down specific PR's in GNATS. 

This removes the current locale caching, and allows num_get and num_put to
work with wide streams.

The patch is pretty big, and is thus compressed.

	(__num_base): Add.
	(__num_base::_S_format_float): Move
	__build_float_format into class __num_base.
	(__num_base::_S_format_int): Move __build_int_format into class
