[gcc-in-cxx] PATCH COMMITTED: Fix up C frontend files for C++


This patch fixes up the C frontend files to compile with C++.  This is
mostly a matter of adding a few casts and changing a few types to
follow C++ strict enum rules.  In c-format.c I had to change a bunch
of initializers to change 0 to STD_C89 (which has the value 0); this
is the bulk of the patch.

This whole patch can go into mainline; I will test that at a later
date if nobody else gets to it first.

This patch fixes one actual bug, in parser_build_binary_op.  We were
comparing a tree code against tcc_comparison.
