[patch c++] Broken build for platforms that does not support POSIX like conditional variables


Hi,

For the target x86_64-pc-mingw32, which does not support POSIX like 
conditional variables, the build process of libsupc++ fails.
There is missing the clause '__GTHREAD_HAS_COND' around the single 
conditional variable controlling all static initializations.

I teseted this for i686-pc-cygwin and for x86_64-pc-mingw32 target. And it 
works fine AFAICS.
