         Mention subtargets of sh-*-elf.  Modify m32c-*-elf target 
         description by adding R8C, M16C processor families.
         Modify h8300-hms for recommneding h8300-*-elf over h8300-hms
         or h8300-*-coff.

Use the present tense for a ChangeLog entry.  I'd suggest:

         * doc/install.texi: Add new targets as h8300-*-elf, sh-*-elf.
         Mention subtargets of sh-*-elf.  Modify m32c-*-elf target
         description by adding R8C, M16C processor families.

You might want to include section names in parenthesis to indicate which
section
  of install.texi you are modifying.  You can find a lot of such
examples in ChangeLog.
