Yes, you should fill in that form and send it to assign@gnu.org.

Your patches seem to have been corrupted by being included in a message 
using RFC2646 format=flowed; both patches and format=flowed use otherwise 
insignificant whitespace for their own purposes, and so are incompatible.  
You need to disable the use of format=flowed when sending patches (or, 
less preferable, send them as attachments).
