This version is very slightly changed - clarified documentation and a 
warning in case the user tries to make a global operator new or 
delete non-default visibility (this breaks the C++ standard plus 
causes a most unpleasant linker abort() if you use an inlined global 
operator new). I don't think this requirement need apply to class 
operators new & delete so it only warns for the global operators.

The copyright assignment for this patch has been completed and is 
currently still in transit back to the US.
