More xm-*.h cleanup

This patch removes from the xm-*.h files all definitions of macros
which I could find no use of anywhere in the source tree, or whose
definition is adequately handled already by the configure script.
With one exception, I did not attempt to do anything about macros
which are used, but shouldn't be:

FUNCTION_CONVERSION_BUG
HZ
IO_BUFFER_SIZE
MAXPATHLEN
OBSTACK_CHUNK_SIZE
POSIX
QSORT_WORKAROUND
USG
__MSDOS__
environ
__i386__
isinf

Most of these should be dealt with by autoconf. If you grep for them,
you'll find some of the worst instances of unregenerate #ifdeffing.
Also, I didn't touch anything to do with alloca or filesystem issues;
both badly need a cleanup, but the ramifications are too big for the
moment.

I also pulled out all the commented-out macro definitions I could
find. This is to get rid of false positives when grepping for
something.

The one exception, mentioned above, is P_tmpdir. mips/mips.c had its
own private routine to create temp files. I killed it in favor of
make_temp_file. That got rid of the one remaining use of P_tmpdir
inside the gcc directory, so the definitions all went too.

Applied as obvious cleanup, to mainline. I compiled mips/mips.c, but
I did not do a full bootstrap on any target.

There are things in the xm-beos.h files which only make sense if you
can't run configure on that host. Is that true?
