major texi2pod update


This update adds considerable functionality to texi2pod.pl; most of it
was requested by Joseph Myers, for his project to generate gcc.1 from
invoke.texi.  It's still not quite capable of doing that, but is a
large improvement over what we had.

Tested by regenerating cpp.1 and gcov.1, then diffing the formatted
manpages against the versions in CVS.  Applied.

	* texi2pod.pl: Restructure for comprehensibility, add
	comments.  Merge handling of @ignore and @ifxxx.  Handle a
	whole bunch more Texinfo commands.  Use consistent formatting
	style.
